How Do I Claim An Offer?
Fill out the form you see on this page. When we speak with you please let us know which offer you'd like to claim. We'll collect payment over the phone as well as get you scheduled for an appointment. If you are purchasing an evaluation or clinic credit for a friend or family member we'll collect payment as well as gather the appropriate contact information for your friend or family member. We will reach out to them promptly.
How Do I Know If I'm A Good Fit For Your Clinic And These Offers?
Once you fill out the form we'll be in touch via phone. If you are a new patient to our practice we will have a complimentary discovery call to hear more about what you're dealing with. We will let you know if we feel that we can help with your issue. If we don't feel that we are the best fit for you we will happily refer you to a clinic that may be a better fit.
What If I Don't Use All Of The Treatment I Purchased?
While all sales are final, treatment does not expire. If you have remaining treatment value after your plan of care concludes the treatment will remain in your account as clinic credit for you to use at a future date.
What If I Purchase Treatment For A Friend Or Family Member And They Don't End Up Using It?
While all sales are final, if your friend or family member doesn't ultimately want to utilize the treatment we can transfer its value back to you to use or you can gift it to another friend or family member and we will happily transfer it to them.
